Are there some simplified instructions available on how to take a Word document and make it fillable on line?
If you have a WordPress web page where users will interact with the form, you can load a “Plugin” such as “Forminator”, which allows you to create a form with a series of fields including multiple choice and free text, and emails you the results. Setting up the form would be a copy-and-paste procedure, copying the prompts from your Word document and pasting them into Forminator. On the other hand, if you just want to email a form and have people email it back to you, that would require Adobe or Microsoft Forms. I have not done this myself. You could also post the form in a shared location such as Google Drive. I’d recommend doing another search in your browser, specifying what products you want to use.
