Anyone know what this could be? I’ve tried scraping them off to no avail and sprayed with neem oil. I want to prevent further damage to this rose, or the chance of it spreading to my other roses! Any help is greatly appreciated!

Welcome to the forum, Katherine, it reminds me of some type of scale, however, the pic is fuzzy and hard to identify the culprit accurately. There are insect id apps you might access that might help you determine what is going on. Also, you might want to contact your local rose society’s ARS consulting Rosarians, or if you live near a state extension office cut the stem, put in water, and let them look at it.
